Overview of Accident:

A plumber was greasing joints in a trench that was seven feet deep. Prior to the trench where the plumber was working another trench had been dug and backfilled adjacent to the trench that collapsed and killed the plumber.

Materials that Need to be Collected:

There are a number of document that need to be collected to attempt to reduce the burden of the general contractor's penalties. The first item needed is the safety plan and level of personal protective equipment required while on the job site. The cause of death on according to the OSHA report was that the employee was struck on the head while working in a trench. All of the laborers that were on the job site should have been required to be wearing hard hats and safety vests.

The next piece of information that needs to be gathered is the Job Hazard Analysis sheets that should have been filled out before starting any new work item. These Job Hazard Analysis sheets need to list all the hazards that might come up during the course of work and the steps taken to protect laborers from these hazards. Each laborer's signature should be on the Job Hazard Analysis sheet as a record that they were made aware of the hazard and how to work safely around the hazard.

Training documents for combined spaces need to be collected for all of the laborers that were working in this area because of the length and depth of the trench.

Past safety records needs to be gathered for both the general contractor and the plumbing subcontractor.

The last document that needs to be gathered in the accident investigation report is the accident report that is required by company policy. The accident report will start with the name of who filled it out (usually the construction manager), the date, time, how the accident occurred, and the effect of the accident on the employee. Each
