Today I had the chance to provide care to a patient who needed her urostomy bag changed, burp her colostomy bag, and help another patient with she was feeling nauseous and getting sick. Observing the nurse I saw his Med Pass routine, discharge of patients, and what how he provides care to patients.Overall I enjoyed being able to provide more care to the patients, and observe the nurse every experience is a learning experience and can be a helpful teaching tool for the future.

How did the nurse interact with other members of the interdisciplinary team?

The nurse had a patient who was discharging today and that required a lot of work with interdisciplinary teams. The doctor needed to get the correct paperwork around for the patient and had asked the nurse to fill in any missing information and print it for him. The nurse also had to work with therapy so the patients wheel chair and all the parts were ordered and ready to go when the patient was discharging. The nurse also worked with the driver who was taking the patient home by ambulance.

What other members of the interdisciplinary team did you observe today?

I observed the assisted personal provide care, therapy, dietary, and the doctor. It is so important that all of the interdisciplinary teams are able to work together, with lack of communication a patients care can vary greatly. For example it was not communicated to the ambulance driver that a patient would need a large chair lift to get into the vehicle and the driver was unable to get the patient to her wound care appointment. Luckily another appointment could be made 3 days later but if not the patient could have suffered from missing it, and her would could have gotten worse.
